Mt. Healthy City Schools will be implementing Final Forms as the new system for student data management. Final Forms allows parents/guardians to enroll students, update contact information, provide emergency medical information, and much more. In addition, Final Forms allows for users to select their native language for ease of use.
An email from Final Forms for enrollment will be sent Friday 8/26 by 12PM. The parents/guardians of students with an email on file should receive this notification. If you do not receive the email from Final Forms, please go to the links below.
